This farm monitoring form for cacao-based cultivation systems was produced by the Sustainable Farming in Tropical Asian Landscapes (SFITAL) Project in the Philippines in line with its establishment of Cacao Agroecology Learning Farms (CALFs). These CALFs are located in four municipalities in Davao de Oro, namely Laak, Maco, Nabunturan, and New Bataan. These farms were established in cooperation with the farmer-owners and local enabling institutions to further promote the application of agroecological practices for climate-resilient and environmentally friendly cacao cultivation systems.
This monitoring form, which can be used for several months, is designed to be hung in the farm or the home the farmer, so that the entries can easily be seen by the farmer, their farm laborers (if any), and the local extensionists coaching or mentoring them (if any). Data are inputted in this form through writing and stickers (printable copies of which are also in this file) to make monitoring and data collection less tedious and more enjoyable.

Moreover, the form is produced in two languages, English and Cebuano. At present the months are arranged according to the duration of the SFITAL Project. But the pages can be re-organized according to the needs of the farm where it will be used.
